To acquire Apostille for your PSA Birth Certificate, a Local Civil Registry FORM 1A may be required by DFA. (see this image for sample)
Please ask a family member to acquire this document from the municipality where your birth was registered.

We only need a signed Authorization letter.
However, if the owner of the document is a minor, we do require Special Power of Attorney.

Yes!
As long as it is issued by the Philippine Statistics Authority (PSA) and not by the old office NSO (National Statistics Office).
Please send the original document to our liaison officers, so we can take the document to the DFA and have it apostillized.
If your PSA document is with you abroad, it might be more costly for you to send that document back to us in the Philippines.
In this case, we suggest that you let us get a new copy from the PSA for you instead.

Max 6 months.
Please double check with the department to which you will be submitting your documents, to verify how recent is the accepted CENOMAR.
In Norway, CENOMAR should not be more than 4 months old.

It is a certificate issued by the Department of Foreign Affairs verifying the authenticity of your document.
